45 out of 51 (counting DC). Not too shabby.
Alaska might be dubious; I've never set foot on Alaskan soil, but I was aboard the Ustafish when we made a very quick port call in Adak. We were met by a small boat out in the harbor, transferred some riders, and went back out to sea.
The one that kind of surprises me is Michigan; but I'm pretty sure I didn't go through there on either of the cross-country drives (and I know that none of my long-distance Amtrak trips went through there).

"Only" 17 countries. Well, that's still a lot more than a lot of folks I know. I do need to get myself down to South America at some point, and to Africa as well (though perhaps not the same trip).
Now, I'm deliberately not counting any ex-Soviet Union territory that the Ustafish may have spent time in international waters just offshore of. OTOH, China gets selected because I did visit Hong Kong.
